Dental Insurance and Payment Options
Financing Your Care
You are the most important factor regarding your health. Recommended treatment will be determined by your individual needs, not your dental insurance benefits. After consulting with Dr. Cavallari, you will meet with one of our Treatment Plan Coordinators who will discuss all treatment recommendations with you.
Dental Insurance Considerations:
Dental insurance is meant to aid a patient in receiving care, not to cover procedures in totality. Most plans base the amount of benefit on a schedule of fees arbitrarily developed by the insurance company. For this reason, you may receive a lower percentage than the reimbursement level indicated in your dental plan.
For example, if your plan states that it will pay 80% of the cost of a specific treatment, it means 80% of the fee arbitrarily determined by the insurance company and not the actual fee charged by our office. Waiting periods and other clauses/exclusions will also affect your coverage.
Please know that we will do everything possible to see that you receive the full benefits of your policy, however, your estimated portion is due and payable in full at the time of your visit, and any remaining balance once claims have been processed, will be your responsibility.
Please note that your insurance policy and its coverage is an agreement between yourself and the insurance company. If you feel your coverage is insufficient and are not privately insured, you will need to address that issue with your insurance company or employer.
As a courtesy to our patients, we will submit and track primary and secondary dental claims associated with treatment provided in our office. In order for us to properly manage your dental care needs, current information is imperative. Please help us keep your records up to date by informing us of any changes to your account. This would include but not be limited to: name, address, phone number, email address, employer, insurance, and all medical/health history.
Payment options:
- Cash or Check: Always accepted.
- Credit Card: Visa, MasterCard, Discover, or American Express.
- CareCredit: Apply online (www.carecredit.com) or in our office.

Please note: Payment is due at the time of service.
As a courtesy to you, we will bill your insurance company and track claims. Please keep us informed of any changes to your insurance plan. You are responsible for the fees charged by our office, no matter what your insurance coverage may be. Most insurance companies should respond to the claim within four to six weeks. Any remaining cost is your responsibility.
